Assistance Application

These questions are designed to collect detailed information about your medical and financial circumstances, helping us determine your eligibility and the appropriate level of assistance. Please answer all questions truthfully and thoroughly.

Your application will be carefully reviewed by the Pink Warriors Foundation Assistance Review Committee. Assistance will be based on the number of applications received and the information provided in your application. Your honesty and integrity are essential throughout this process. The review process will take approximately 2-3 weeks after receipt of your application.

If your application is approved for a grant award, you will receive notification via email or USPS Mail. One-time assistance will be provided either as a grocery gift card, gas card, transportation voucher, or direct payment to the bill servicer, up to a maximum amount of $250.

One award per year per household

Application Rules;

1. Ensure all sections of the application form are filled out accurately and completely.

2. Sign your name clearly and legibly.

3. Verify that all information provided is truthful and accurate to the best of your knowledge.

4. By signing, you authorize the review committee to access and verify the information provided.

5. Understand that your typed signature indicates consent for the Pink Warriors Foundation to use the provided information for the purpose of evaluating your application.

6. Provide a valid contact method (email and/or postal address) where notification regarding your application status can be sent.

7. Include the date of your signature to indicate when the application was signed.

8. Submit the signed application by the specified deadline.

9. Acknowledge that compliance with these rules is necessary for the processing and consideration of your application.
